Regulatory Brief · Healthcare & Life Sciences · 14 September 2026

Saudi Arabia registered Frehemgo before any other regulator, through a programme that only accepts medicines no one else has approved. A sponsor can now choose to launch in the Kingdom first.

On 14 September 2026 the Saudi Food and Drug Authority became the first regulator in the world to register Frehemgo (denecimig), Novo Nordisk's weekly-to-monthly haemophilia A prophylaxis. Novo filed in the US and EU in September and October 2025; SFDA granted breakthrough designation on 24 May 2026 and approved 113 days later on our count. The first-in-world result is built into the route: SFDA's Breakthrough Medicines Program admits only products registered with no other authority, and lets access negotiations run alongside review. For a sponsor, Saudi Arabia can now be sequenced first on purpose. For a distributor or specialty-pharmacy buyer, launch dates in agreements written on an FDA-or-EMA-first assumption need re-reading.

The 30-second read

What a board member needs before the next meeting on this.

  1. Saudi Arabia approved first while the FDA and EMA were still reviewing. Novo Nordisk filed denecimig in the US and EU in September and October 2025. SFDA registered it on 14 September 2026 as the first authority globally, and Novo's H1 2026 report still listed US and EU decisions as 2026 milestones.
  2. The first-in-world result is a rule of the programme. SFDA's Breakthrough Medicines Program guide makes it an eligibility criterion that the product is not registered at any regulatory authority when designation is requested. A product enters the route unapproved everywhere, so a quick SFDA review puts the Kingdom first.
  3. The clock is short and access talks run in parallel. The guide sets a normal 30-working-day designation review, a 60-working-day scientific assessment, and lets the company start access negotiations with healthcare entities while review is under way. Frehemgo went from designation to registration in 113 days on our count.
  4. Agreements keyed to FDA or EMA approval now carry a timing gap. A distribution or specialty-pharmacy agreement that dates Saudi launch from a reference-market approval can undercount the Saudi revenue window when a sponsor uses this route.

On 14 September 2026 the Saudi Food and Drug Authority approved the registration of Frehemgo (denecimig) for routine prophylaxis of bleeding episodes in haemophilia A, with or without factor VIII inhibitors, and stated that it is the first regulatory authority globally to do so (SFDA, 14 September 2026). Frehemgo is a bispecific antibody injected under the skin once a week, every two weeks or once a month, and SFDA based its decision on four Phase 3 studies.

Its sponsor, Novo Nordisk, had filed the same product in the United States and the European Union a year earlier. Read as a headline, the approval says Riyadh moved faster. Read against SFDA's own rules, it says a sponsor chose to put Saudi Arabia first, and the route it used makes that choice available again.

01

The FDA and EMA had the file first

Novo Nordisk's report for the first nine months of 2025 states that in September and October 2025 it submitted the product, then called Mim8, for regulatory approval in the US and the EU, on data from the FRONTIER programme (Novo Nordisk 9M 2025 report, SEC Form 6-K).

Its half-year report of 4 August 2026 lists a US and EU decision on denecimig among its anticipated 2026 regulatory milestones and records a new submission to China's Center for Drug Evaluation (Novo Nordisk H1 2026 report, SEC Form 6-K). On 24 May 2026 SFDA granted Novo Nordisk breakthrough designation for Frehemgo, stating that the designation was not a marketing authorisation (SFDA, 24 May 2026). On our count, registration followed 113 days after designation, while the US and EU files were still open.

02

The programme only admits drugs approved nowhere else

SFDA's Breakthrough Medicines Program guide, DS-G-109-V01, issued and implemented on 26 October 2023, sets four eligibility criteria that must all be met (SFDA Breakthrough Medicines Program guide). The product must target a serious or life-threatening condition with unmet need, be likely to offer a major advantage over current methods, and carry an expected positive benefit-risk balance.

The fourth criterion is that it is not registered at any regulatory authority when the designation is requested. The first-in-world approval is therefore the design of the route. What changes is that a large sponsor has now used it on a product with pending US and EU files, and SFDA registered it before either of them decided.

03

The clock and the access talks

The guide gives the timetable a sponsor can plan against. SFDA's scientific committee normally reviews a designation request within 30 working days. Where the data are complete, the sponsor must submit the application within 30 working days of designation and may ask to be exempted from requirements it cannot yet meet, naming the certificate of pharmaceutical product, leaflet and artwork.

The scientific assessment takes 60 working days, and the sponsor has 60 working days to answer any queries. The guide also states that the company can start medication access negotiations with all involved healthcare entities in parallel with review. Saudi pricing and access talks can therefore open before any reference-market price exists. SFDA has granted designation to other products under the same programme, including Bristol-Myers Squibb's ZENBEXUS (iberdomide) on 4 June 2026 (SFDA, 4 June 2026).

04

What it does to agreements and valuations

A GCC distribution or specialty-pharmacy agreement written on the assumption that Saudi registration trails an FDA or EMA approval will often run launch dates, exclusivity periods, minimum volumes and marketing commitments from that trigger. Where a sponsor uses the breakthrough route, the Saudi window opens first, and terms keyed to a reference-market event can leave months of revenue unaccounted for on one side of the contract.

For a buyer of a distributor or specialty pharmacy, the pipeline forecast should date each product's Saudi revenue from the route it will actually take. The same pattern appeared in the UAE this year, where the Emirates Drug Establishment cleared two products first or second worldwide, as we set out in our EDE launch-sequencing brief.

05

What the approval does not settle

Registration is not coverage. SFDA's notice says nothing about price, formulary listing or procurement, and those remain separate negotiations with Saudi payers and buyers.

Before your next meeting

Four questions before the next Saudi launch decision

  1. Which assets in our pipeline treat a serious condition with unmet need and will still be unregistered everywhere when we could request SFDA breakthrough designation?
  2. If we take that route, who opens access and price negotiations with Saudi healthcare entities during the review, and on what reference price?
  3. Which of our Saudi and GCC distribution agreements date launch, exclusivity or minimum volumes from an FDA or EMA approval, and what happens to them if Saudi registration comes first?
  4. For a distributor or specialty pharmacy we might buy, which products in its pipeline hold or could seek SFDA breakthrough designation, and does its forecast date their Saudi revenue from SFDA or from Washington and Amsterdam?

The Monday work is specific. List pipeline assets that could meet all four criteria while still unregistered everywhere, and decide for each whether Saudi Arabia goes first. Pull every Saudi and GCC distribution agreement and mark the clauses that run from an FDA or EMA approval. In any distributor or specialty-pharmacy diligence, ask which pipeline products hold or plan SFDA breakthrough designation. That sequencing work is where our market access and regulatory practice begins.
